Child Nutrition can add a dietary alert to a student's meal account and provide meal accommodations if it is due to a life threatening food allergy or other medical condition/disability that affects a student's participation in the school meals program.  The department must receive a completed medical statement (dietary) form from a physician that is licensed by the state to write prescriptions.

Below are the links to the CNS medical statement (dietary) form.

A Note Regarding Peanuts:
Elementary: We do not serve peanut products in elementary schools, but we cannot guarantee that foods we purchase have not been in contact with nuts during manufacturing.
Secondary: The Humble ISD Child Nutrition Department offers Chick-fil-A sandwiches in some secondary schools. The Chick-fil-A chicken patties are cooked in peanut oil.
